Gencydo® in der Therapie dissoziativer Symptome. Eine longitudinale, prospektive Anwendungsbeobachtung bei psychiatrischen Patienten

Hintergrund: Obwohl dissoziative Symptome bei psychiatrischen Patienten mit verschiedensten Diagnosen eine große Rolle spielen, bestehen zu ihrer Therapie bisher keine einheitlichen Empfehlungen. Seit etwa 70 Jahren findet im Rahmen Anthroposophischer Medizin ein Extrakt aus Citrus limon und Cydonia oblonga (Gencydo®) mit kasuistisch vielfach beschriebener Wirksamkeit Verwendung; es liegen hierzu jedoch keine systematischen Untersuchungen vor. Patienten und Methoden: In einer Pilotstudie wurde in einer systematischen, prospektiven, longitudinalen Anwendungsbeobachtung erstmals die Injektionstherapie mit Gencydo bei 21 stationär-psychiatrisch behandelten Patienten mitdissoziativen Symptomen untersucht. Es wurden kurz- und langfristig die Ausprägung dissoziativer Symptome evaluiert sowie Verträglichkeit und Praktikabilität einer Gencydo-Therapie untersucht. Ergebnisse: Im kurzfristigen Verlauf zeigte sich nach GencydoInjektion eine Zunahme der Körper- und Umgebungswahrnehmung. Langfristig nahm die dissoziative Symptomatik bei 15 von 21 Patienten ab. Diese positiven Effekte waren bei Patienten ohne Psychopharmako-Therapie sowie bei Patienten ohne Persönlichkeitsstörungen häufiger und ausgeprägter. Schlussfolgerungen: Die vorliegenden Ergebnisse deuten auf eine zumindest akute und kurzfristige, möglicherweise auch längerfristige Wirksamkeit von Gencydo bei Patienten mit dissoziativen Symptomen hin. Das Medikament erweist sich als gut verträglich und sicher in der Anwendung. Weitere Untersuchungen an umschriebenen Patientengruppen in einem kontrollierten Studiendesign sind zur konfirmatorischen Prüfung der Wirksamkeit von Gencydo nötig.

Background: Despite the considerable role played by dissociative symptoms in psychiatric patients with various diagnoses, there are currently no standard guidelines for their treatment. An extract from Citrus limon and Cydonia oblonga (Gencydo®) has been used, with numerous case records demonstrating its efficacy in anthroposophical medicine for approximately 70 years, but no systematic investigation of the drug and its efficacy has so far been done. Method: In a systematic, prospective, longitudinal observational pilot study, injection therapy with Gencydo was investigated for the first time in 21 psychiatric in-patients with dissociative symptoms. Subject to investigation were the short and longterm severity of dissociative symptoms, and the tolerability and practicability of Gencydo therapy. Results. Following Gencydo injections, short-term increases in awareness of body and environment were observed. Long-term reductions in dissociative symptoms were found in 15 of the 21 patients. These positive effects were more frequent and greater in patients not given antipsychotic therapy and in those without personality disorders. Conclusions: Results indicate acute short-term and possibly also long-term effectiveness of Gencydo in the treatment of patients with dissociative symptoms.The drug proved to be well-tolerated and easy to administrate. Control design studies with specific patient groups are required to confirm the effectiveness of Gencydo in the treatment of dissociative sym ptoms.
