Anthroposophische Therapeutische Sprachgestaltung bei Asthma bronchiale: eine prospektive, kontrollierte multizentrische Crossover-Studie

Atemschulung gewinnt bei der Behandlung von Asthma bronchiale international an Bedeutung. Wir untersuchten die Wirkung von Anthroposophischer Therapeutischer Sprachgestaltung (ATS) als Atemtherapie im ambulanten Setting. In dieser randomisierten kontrollierten Crossover-Studie erhielten Patient*innen in drei Zentren in der Schweiz und in Deutschland entweder zuerst 11 ATS-Sitzungen oder warteten. Insgesamt schlossen 49 Personen die Studie ab. Statistisch signifikante Veränderungen zeigten sich im AQLQ-Gesamtscore (AQLQ: Asthma Quality of Life Questionnaire) und in den Domänen: Symptome, Aktivitätseinschränkung und emotionale Funktion sowie bei der Asthmakontrolle (ACT). Für Spirometrie, inhalative Glukokortikoide, Peak-Flow und Tage ohne Exazerbation fanden sich keine signifikanten Unterschiede. Während der ATS-Sitzungen traten keine ernsthaften unerwünschten Ereignisse auf. Die Analyse der zusätzlichen Freitextkommentare von Teilnehmenden, Ärzt*innen und Therapeut*innen weist auf Probleme des Studiendesigns und Verbesserungen in Domänen wie Stimmqualität und ATS als Instrument zur Selbsthilfe hin. ATS verbessert signifikant die Asthmakontrolle und Lebensqualität bei Asthma bronchiale. Mögliche Verbesserungen der Lungenfunktion durch ATS bleiben offen. Die Studie wurde am 8. Juli 2015 unter ClinicalTrials. gov NCT02501824 retrospektiv registriert.

Anthroposophical Therapeutic Speech (ATS) in asthma: a prospective randomized, controlled multicentre crossover trial

Breathing retraining receives increased attention in the management of asthma. We investigated the effects of Anthroposophical Therapeutic Speech (ATS) in a real-life outpatient setting. In this randomized controlled crossover trial, outpatients in three centers in Switzerland and Germany were randomized to receive 11 ATS sessions or to wait. 49 Patients completed the study. Significant differences between ATS and waiting groups were found for the overall AQLQ score and the domain scores for symptoms, activity limitation and emotional function as well as for the ACT score. No significant differences were observed in spirometry, inhaled glucocorticoids, peakflow and asthma exacerbations. No serious adverse events occurred. Free-text comments by participants, doctors and therapists revealed improvements in additional domains such as voice quality, self-management and sleep, but problems with the study design. ATS significantly improves asthma control and quality of life in patients with asthma. Whether ATS improves lung function remains to be shown. Trial Registration: ClinicalTrials. gov NCT02501824.
